Tuberculosis remains one of the oldest yet most persistent infectious diseases, and despite global progress, it continues to account for significant morbidity and mortality. Modern efforts emphasize early recognition, drug resistance management, and improved adherence strategies to combat both traditional and multidrug-resistant strains. Breakthroughs in tuberculosis diagnosis and treatment have accelerated progress, with molecular tests, line probe assays, and next-generation sequencing allowing rapid identification of resistance patterns and enabling targeted therapeutic choices. In treatment, shorter multidrug regimens and newer agents like bedaquiline and pretomanid have transformed care for patients with resistant forms of the disease. Host-directed therapies and adjunctive immunomodulators are under exploration, aiming to complement antimicrobial action by enhancing immune defense and reducing tissue damage. Technology also plays a central role; digital adherence tools such as video-based directly observed therapy support patient compliance in resource-limited and urban settings alike. Preventive measures, including prophylactic therapy for high-risk populations and continued vaccine research, strengthen the global strategy against tuberculosis. Multidisciplinary collaboration, community-based awareness programs, and integration of care into primary health systems are crucial to sustaining progress. Addressing social determinants of health, such as nutrition, housing, and access to medical services, remains equally essential. The combined advances in tuberculosis diagnosis and treatment highlight the power of coordinated scientific, clinical, and public health efforts to eventually eliminate this ancient disease
